So many bugs and very non responsive app. For those who cannot get this application to work, please enter your details and upload the files on the pass track website, rather than wasting your time on this application. Once done on the website, you can login to the app and everything uploaded will be there. My COVID test is a two page pdf. Even if I convert it into JPEG, I can only upload one of the pages. Almost always, I have found the server to be down. Very poor maintenance. It also does not allow to save the information. So, if you find that the server is down and you want to try again later you will have to refill the entire form.
